These guides and forms are used by registered medical practitioners to notify the regulator on results of health monitoring.

Demolition work means work to demolish or dismantle a structure or part of a structure that is load-bearing or otherwise related to the physical integrity of the structure, but does not include:
Exemption request
A person who has an obligation under the Work Health and Safety (General) Regulations 2022 or the Work Health and Safety (Mines) Regulations 2022 may submit a request to the regulator to be exempted from fully complying with the requirement of these regulations.

Plant design and registration
The requirement to register items of plant and plant designs serves to ensure that items of plant and plant designs have been inspected by a competent person and are safe to operate.
Item of plant and plant design differ. Design registration applies to a concept; any number of individual items can be manufactured to the same design. Item of plant registration applies to a specific item; each item of the specified type requires registration.
Serious injury or illness
The regulator must be notified of any serious injury or illness arising through the conduct of a business or undertaking (PCBU) by calling us on 1800 678 198 (24 hours).
General workplaces (not a mine or a petroleum/geothermal operation)
You need to lodge information online even after notifying by phone.

MON 1: Notice of information about mine operator
Notices for becoming a mine operator at a mine, changing the mine operator at a mine or updating details of a mine operator.
This applies at mine where mining is conducted and also where only exploration operations are conducted.
MON 2: Notice of information about non-exploration mining operations
Notice for commencing mining operations or additional mining operations at a mine [r. 675UJ(2)]
Regulation 675UD of the Work Health and Safety (Mines) Regulations 2022 requires information about commencement of additional mining operations to be notified to the regulator.
